Off-Label and Repurposed Medicine: What It Actually Means
If you've looked into ivermectin, hydroxychloroquine, or mebendazole outside their original approvals, you've run into the phrase “off-label.” It sounds risky. It's worth understanding what it actually describes.
What “off-label” means
The FDA approves a drug for a specific use after reviewing evidence for that use. “Off-label” simply means a licensed physician prescribes an approved medication for a different purpose, dose, or population than the one on the label. It is legal, common, and a normal part of clinical practice — studies estimate a meaningful share of all prescriptions are written off-label.
Off-label does not mean unapproved, experimental, or unregulated. The medication itself has still gone through FDA review for safety and manufacturing. What's “off-label” is the specific use — and the responsibility for that judgment sits with the prescribing physician.
What it does not mean
- It does not mean a medication is proven to work for the off-label use. Evidence varies a great deal from one use to the next.
- It does not mean the decision is casual. A physician still weighs your history, other medications, and risks.
- It does not mean a prescription is guaranteed. If an off-label use doesn't make clinical sense for you, the right answer is no.
